Dental Medicaid credentialing. State enrollment first, then every plan.

Medicaid credentialing for dentists has two layers: enrollment with the state Medicaid program, then credentialing with each Medicaid dental plan. Miss the first and the second does not count. We run both for Florida, California (Medi-Cal Dental) and New York, from the application to the effective date.

The 30-second answer

To see Medicaid patients, a dentist enrolls with the state Medicaid program first, then credentials with each Medicaid dental plan. Managed care does not skip the state step: federal rules require every Medicaid network provider to be enrolled with the state. Florida uses its online enrollment wizard and lists 60 days or less; California uses PAVE and allows up to 180 days; New York uses its Provider Services Portal, currently under 20 days. Individual dentists pay no application fee in any of the three.

Why Medicaid credentialing has two layers

The state enrollment comes first, even for managed care. Under the 21st Century Cures Act and 42 CFR 438.602, every provider in a Medicaid managed care network must be screened and enrolled with the state. Dental plans check the state's enrolled and pending lists; a dentist who never started state enrollment gets claims denied or is removed from the network. Then each dental plan credentials you on its own terms: LIBERTY, for example, checks license, DEA, malpractice ($1M/$3M in Florida) and federal exclusion lists, and recredentials every 36 months.

    Florida Medicaid dental enrollment

Where you apply: Online Provider Enrollment Wizard on the Florida Medicaid Web Portal (portal.flmmis.com)

Paths: Full enrollment (fee-for-service and managed care); Limited enrollment (managed care plan networks only).

Timing and fee: AHCA lists 60 days or less from a complete application; confirm on AHCA's provider pages. No fee is named for dentists in Florida's enrollment policy.

The plan layer: All Florida Medicaid dental runs through the SMMC prepaid dental plans: DentaQuest of Florida and LIBERTY Dental Plan of Florida. Credential with each plan after (or while) the state enrollment is pending.

    • Fix any deficiency AHCA flags within 21 days, or the application is denied and you start over.
    • LIBERTY can keep a dentist in network for only 60 days while AHCA enrollment is pending, then must terminate and may recoup payments.
    • Dental groups must be dentist-owned.
    • Renew every 5 years; without renewal neither fee-for-service nor the plans pay.
    • Report changes (address, ownership, bank) within 30 days.

Once you are enrolled, see our Florida Medicaid dental billing guide for claims, prior authorization and denials.

California (Medi-Cal Dental) Medicaid dental enrollment

Where you apply: PAVE (Provider Application and Validation for Enrollment), pave.dhcs.ca.gov. Paper applications ended October 31, 2022.

Paths: Billing provider (individual or group) plus each rendering dentist; Ordering, referring or prescribing (ORP) only; Managed care only, through a Dental Managed Care plan.

Timing and fee: DHCS states up to 180 days, or 150 days for applicants who qualify as Preferred Provisional Providers. Returned applications restart the wait. The 2026 federal application fee ($750) applies to institutional providers; individual dentists and ORP applications are exempt. Ask DHCS whether your group entity owes it.

The plan layer: Most of California is fee-for-service Medi-Cal Dental. Dental Managed Care runs only in Sacramento (mandatory) and Los Angeles (members opt in), with LIBERTY Dental Plan, Health Net and California Dental Network as the plans from July 1, 2025. Each plan credentials you separately.

    • A returned application must be resubmitted within 60 days.
    • No payment for any service before enrollment is active.
    • Since June 26, 2026, pharmacy claims from prescribers who are not enrolled are denied, so prescribing dentists need at least an ORP enrollment.
    • Revalidation goes through PAVE when DHCS sends the notice; do not submit early.

Once you are enrolled, see our Denti-Cal billing guide for claims, prior authorization and denials.

    New York Medicaid dental enrollment

Where you apply: NYS Medicaid Provider Services Portal (PSP), emedny.org/PSP. Paper processes ended in May 2026.

Paths: Option 1: billing fee-for-service provider; Option 2: non-billing (ordering/referring, or Managed Care Network Provider); Option 3: convert a non-billing enrollment to billing.

Timing and fee: NY DOH reported in August 2026 that PSP applications are processing in under 20 days (paper took 90 to 120). No application fee for individual dentists or dental groups.

The plan layer: New York Medicaid dental is a managed care benefit that plans subcontract to dental vendors such as Healthplex, DentaQuest (EmblemHealth moved to DentaQuest on September 1, 2025), LIBERTY, Delta Dental and Dental Benefit Providers. Check each plan's current vendor before you credential; the state's published vendor list dates from 2022.

    • Applications started but not submitted are deleted after 45 days.
    • No reply to a DOH information request within 45 days withdraws the application.
    • After a revalidation notice you have 123 days, or the enrollment is terminated; every provider is due by June 2028.
    • Dentists who are not enrolled are removed from Medicaid managed care networks.

Once you are enrolled, see our New York Medicaid dental billing guide for claims, prior authorization and denials.

Fees and the 2026 revalidation push

The federal Medicaid application fee for 2026 is $750 (it was $730 in 2025). It applies to institutional providers, not individual practitioners, and it is waived if you have already paid it to Medicare or another state's Medicaid or CHIP program. None of the three states charges individual dentists an enrollment fee.

Revalidation is speeding up. Federal rules require it at least every 5 years, and in 2026 the states began planning faster cycles: New York is revalidating every provider by June 2028 and gives 123 days after a notice before termination. A lapsed revalidation stops both fee-for-service and plan payments, so revalidation dates belong on the same calendar as license and malpractice renewals.

    Ori Bekerman, Founder

Built and scaled a multi-location dental organization. Every new provider meant credentialing with every payer at every location. The paperwork, follow-ups, and timeline tracking were never handled well in-house.

PracticeAlpha was built to fix that. Credentialing managed as a revenue-critical process, not background paperwork.

    Dental Medicaid credentialing FAQ

Do I need to enroll in Medicaid if I only join a Medicaid dental plan?

Yes. Under the 21st Century Cures Act and 42 CFR 438.602, every Medicaid managed care network provider must be screened and enrolled with the state. Florida offers a limited enrollment for managed-care-only dentists, New York a non-billing Managed Care Network Provider option, and California lets plans enroll managed-care-only providers. Dentists who never enroll get removed from plan networks or have claims denied.

How long does Medicaid dental enrollment take?

Florida's agency lists 60 days or less from a complete application. California's DHCS allows up to 180 days, or 150 for preferred provisional providers. New York reported PSP applications processing in under 20 days in August 2026. Plan credentialing comes on top of that.

Is there an application fee for dentists?

Not for individual dentists in these three states. The 2026 federal Medicaid application fee is $750, but it applies to institutional providers, and New York states no fee is required for dentists or dental groups. In California, ask DHCS whether your group entity owes it.

Which dental plans do I credential with in Florida?

Florida Medicaid dental runs through the SMMC prepaid dental plans, DentaQuest of Florida and LIBERTY Dental Plan of Florida. LIBERTY can keep a dentist in network for only 60 days while AHCA enrollment is pending.

    What is PAVE?

PAVE (Provider Application and Validation for Enrollment) is California's online Medi-Cal enrollment portal. Since October 31, 2022, all Medi-Cal Dental enrollment, changes and continued enrollment go through PAVE e-forms instead of paper.

Does California Medi-Cal Dental use managed care?

Only in Sacramento County (mandatory) and Los Angeles County (members opt in). Since July 1, 2025 the Dental Managed Care plans are LIBERTY Dental Plan, Health Net and California Dental Network. Everywhere else is fee-for-service Medi-Cal Dental.

What does a New York dental group need that individuals do not?

An IRS SS-4 or assignment letter (a W-9 is not accepted), NYSED business entity registration for a PC or PLLC, and the Dental Group Provider Information Request, in addition to each dentist's own enrollment.

How often do I revalidate Medicaid enrollment?

At least every 5 years under federal rules. Florida renews non-institutional providers every 5 years. New York uses risk-based cycles and is revalidating every provider by June 2028, with 123 days to respond after a notice. California revalidates through PAVE when DHCS sends the notice.

Why do Medicaid dental applications get delayed or denied?

Missing documents and missed deadlines. Florida denies applications whose deficiencies are not fixed within 21 days, California returns incomplete applications that must be resubmitted within 60 days, and New York deletes unsubmitted applications after 45 days and withdraws ones whose information requests go unanswered for 45 days.

Can PracticeAlpha handle Medicaid enrollment and the plan credentialing?

Yes. We prepare and submit the state enrollment, track every deadline, then credential you with each Medicaid dental plan and confirm effective dates in writing, so claims go out in network from the first visit.
