Guardian's own processing tends to run about 30 to 60 days, which is on the faster end for a major carrier, though the full end-to-end timeline still commonly lands in the 60 to 90 day range once you count profile prep and any back-and-forth. Because Guardian moves reasonably quickly when the file is clean, the practices that lose time are the ones that start with a stale CAQH profile. Starting 90 to 120 days ahead of a new associate or practice opening leaves room for the whole cycle.
