Anthem credentialing pulls from CAQH ProView, so the first job is a complete, current, and attested CAQH profile with every field correct: identifiers, addresses, phone, email, and current malpractice, license, and DEA on file. A single stale field is enough to send the application back.

Plan on 60 to 120 days from a clean submission to approval, depending on documentation and the state. Because Anthem is the Blue plan in its 14 states, groups operating across state lines credential with Anthem in one state and a different Blue plan in another.
