Full periodontal charting with pocket depths is required. Payers want to see probing depths of 4mm+ to justify SRP. If you submit D4341 without charting showing pockets, the claim gets denied. Chart before you treat.
Most plans cover SRP once per quadrant every 24 months. If the patient had SRP on the same quadrant less than 2 years ago, it will likely be denied. Some plans allow exceptions with documentation of disease recurrence.
Bone loss visible on radiographs strengthens the claim significantly. Payers look for radiographic evidence of periodontal disease in addition to clinical findings. Submit current radiographs with the claim.
D4341 and D1110 cannot be billed on the same quadrant on the same date. SRP and prophylaxis are different procedures for different conditions. Billing both looks like double-dipping.
Six-point probing depths for all teeth. Pocket depths of 4mm+ in the treated quadrant to justify the procedure.
Current radiographs showing bone levels. Bone loss supports the periodontal diagnosis.
Periodontal diagnosis documented: localized or generalized periodontitis, stage and grade if using current classification.
If the payer requests, provide a narrative explaining the extent of disease and treatment rationale.
